Facilities Use Form
Any individual, group, or organization wishing to use the facilities of the Grandview Heights City Schools must complete and sign a Facilities Use Form. |

Acceptable Use and Internet Safety Policy
On August 8, 2007, the Board of Education adopted the Acceptable Use and Internet Safety Policy for Students. Please read the policy with your child. Sign and return the policy to your child's homeroom teacher or school office. |

Right to Privacy Form
Each fall, student rosters are prepared by the school offices. The information on the roster includes the child's name, parents' names, address and phone number (unless specified unlisted). This information is prepared by the PTO and used by the school office.
Federal law requires that we give families the option of having this directory information deleted from the roster. An additional aspect of the law requires parental permission for use of a child's photograph in any and all press releases publicizing school activities.
Please complete the Right to Privacy Act form and return it to the office. |

Volunteer Sign-up
If you are interested in volunteering at the Edison Intermediate Middle School, please complete and return the Volunteer Sign-Up form. |

Walking Permission Form
On some occasions we will walk to places in our community. By completing the Walking Permission Form, your child will be able to participate in these walks. |
